University Mentor Daily Record

The daily mentor record will assist you in maintaining accurate records as well as help in compiling your timesheet at the end of the month.  If you are unable to go to your assigned school on a specific date, you MUST call the advisor to let them know and call our office (303.492.7601) and leave a message.
